At Guided Path Senior Care, our goal is to provide compassionate, ethical, and professional nursing care while ensuring the safety of every client. Please review the following policies before beginning services.
Physician Orders for Skilled Nursing Services
Guided Path Senior Care provides care management, patient advocacy, health education, wellness visits, and nursing oversight as private-pay services.
Certain skilled nursing procedures require a physician’s order and may also require that they be performed under the direction of a licensed healthcare provider.
Examples include, but are not limited to:
- Wound care requiring skilled nursing treatment
- IV medication administration
- IV hydration
- PICC line and central line care
- Catheter insertion or replacement
- Injectable medications when a physician order is required
- Tube feeding management
- Any skilled nursing service that requires physician authorization under Ohio law
If physician orders are not available, these services cannot be performed.
If appropriate, we are happy to coordinate with your physician to obtain the necessary documentation.
Private-Pay Services
Guided Path Senior Care is a self-pay (private-pay) practice.
We do not accept Medicare, Medicaid, or private insurance.
Payment is due according to the terms outlined in your service agreement.
Scope of Services
Our services are designed to complement-not replace-your physician or emergency medical care.
We do not:
- Diagnose medical conditions
- Prescribe medications
- Replace emergency medical services
- Provide continuous bedside nursing care
- Make medical decisions on behalf of clients
Clients experiencing a medical emergency should call 911 or seek immediate emergency care.
Emergency & After-Hours Care
Guided Path Senior Care is not an emergency medical service and does not provide 24-hour, overnight, or after-hours nursing care.
If you or your loved one experiences a medical emergency—including chest pain, difficulty breathing, signs of a stroke, uncontrolled bleeding, or any other life-threatening condition—call 911 immediately or go to the nearest emergency department.
Messages received outside of normal business hours will be returned during the next business day. If an urgent medical concern arises after hours, please contact your physician’s office, utilize their after-hours on-call provider, visit an urgent care center, or seek emergency medical care when appropriate.
Our services are intended to provide ongoing care management, nursing guidance, patient advocacy, and support—not emergency or crisis intervention.
Client Eligibility
Before services begin, every client completes a comprehensive RN assessment to determine whether our services are appropriate and safe.
Right to Decline or Discontinue Services
Guided Path Senior Care reserves the right to decline or discontinue services when necessary to ensure the safety of our clients, staff, and the integrity of our practice.
Examples may include:
- Requested services fall outside our scope of practice
- Required physician orders cannot be obtained
- The client’s needs exceed what can safely be provided in the home
- Unsafe home environment or unsafe working conditions
- Verbal abuse, threats, harassment, discrimination, or violence toward staff
- Non-payment for services
- Repeated missed appointments or failure to participate in the agreed-upon plan of care
Whenever possible, we will provide referrals to other appropriate resources or healthcare providers.
Client Rights
Every client has the right to:
- Be treated with dignity, respect, and compassion
- Participate in decisions regarding their care
- Receive services without discrimination
- Have personal health information kept confidential in accordance with HIPAA
- Ask questions and receive clear explanations regarding services
Professional Practice Statement
Guided Path Senior Care provides private-pay Registered Nurse care management services in accordance with the Ohio Nurse Practice Act. Services are provided within the scope of nursing practice and in collaboration with the client’s healthcare providers when appropriate.
We reserve the right to refuse or discontinue services when:
- Requested services fall outside the scope of our practice.
- Appropriate physician orders cannot be obtained for skilled nursing services.
- The client’s needs exceed what can be safely managed in the home setting.
- The home environment is unsafe for staff or clients.
- There is verbal abuse, harassment, threats, discrimination, or violence toward our staff.
- Payment obligations are not met.
- The client or responsible party repeatedly fails to participate in the agreed-upon plan of care.
These policies are in place to protect both our clients and our nursing staff while ensuring we can provide safe, ethical, and high-quality care.
