Please Note: Guided Path Senior Care is a private-pay service. We do not accept Medicare, Medicaid, or private insurance. This allows us to provide flexible, individualized care without the limitations often associated with insurance-based services.


New Client Assessment

Every client begins with a comprehensive Registered Nurse assessment. This initial visit allows us to understand your unique needs and create a personalized plan of care.

During your assessment, we will:

  • Complete a comprehensive health and wellness evaluation
  • Review all medications for safety and accuracy
  • Evaluate your home for potential safety risks
  • Assess fall risks and mobility concerns
  • Meet with family members to discuss goals and concerns
  • Develop individualized recommendations to improve safety, health, and quality of life

Following your assessment, we’ll recommend the level of support that best fits your needs.


Ongoing Care Management Services

Our care management services are designed to provide continuous support while helping seniors remain as independent as possible.

Services may include:

Wellness Monitoring

Regular nursing visits to monitor overall health, identify changes early, and help prevent unnecessary hospitalizations.

Medication Review & Oversight

Medication reconciliation, organization, education, and monitoring to promote safe medication use.

Care Coordination

Communication and coordination with physicians, specialists, home health agencies, therapy providers, pharmacies, and other healthcare professionals.

Physician Communication

We help ensure important health concerns are communicated clearly with your medical providers and assist in coordinating follow-up care.

Family Communication

Routine updates to family members, especially those who live out of town, providing peace of mind and keeping everyone informed.

Home Safety Evaluations

Recommendations to improve safety, reduce fall risks, and support aging in place.

Hospital & Rehabilitation Transition Support

Assistance before and after hospitalizations to help ensure a smooth transition home, reduce readmissions, and coordinate follow-up care.

Personalized Care Planning

Development of individualized care plans that evolve as your needs change.

RN Support

Direct access to an experienced Registered Nurse for guidance, education, and ongoing support.


Additional Services

In addition to ongoing care management, we also offer individual services for clients who need assistance without enrolling in an ongoing care plan.

Guided Path Senior Care operates as a self-pay (private pay) care management practice.

We do not accept Medicare, Medicaid, or private insurance.

This allows us to spend the time your family deserves, provide truly personalized care, and focus on your individual needs rather than insurance limitations. Our services can often complement home health, hospice, therapy, and other healthcare services by providing advocacy, coordination, and ongoing nursing oversight that insurance typically does not cover.
